This is a really really smart idea, simple but really smart.
Doc Ready allows people to create a list of things they want to talk to the doctor about. It is aimed at young people with mental illness, but I think the idea works for anyone who needs to go and see a doctor.
The Doc Ready site gives lots of helpful advice to people who are going to see their doctor for the first time to discuss their mental wellbeing. It covers everything from what happens when you see the doctor through to what your rights as a patient are.
What I really like about it is the checklist that you can build to help you when you are in the appointment.
You start your checklist by selecting one of 15 categories that is affecting you and then you add more detail in. You can go back and add further categories and sub sections before you create your checklist which you can print off, E-mail or just download it as a PDF. Your checklist has space for you to fill in details for your appointment and a hand notes section where you can write down things you need to remember while you are with the doctor.
